It is very important that you notify the Practice if you have moved. We require your current address details, so that we may contact you regarding your personal health.
If you do not inform the Practice of your correct address you may be removed from the list.
Our local Health Board routinely asks GP Practices to check patient addresses to establish if they are still currently registered at the address provided by the patient. If your GP Practice is unable to confirm your details, then the Health Board may remove you from the Practice List.
If you have moved then please use the form below to inform the Practice. We will then update your medical records accordingly. Alternatively you can telephone your change of address to the staff on 0141 334 6111.
